SAWS notified Jiangsu Shuangma Chemical "4.16" dust explosion accident

**Notice of the "4·16" Dust Explosion Accident** **National Security Supervision Bureau** **Nantong City, Jiangsu Province – Rugao Shuangma Chemical Co., Ltd.** **Safety Supervision Manager No. 34 [2014] 34** To all provincial and municipal safety supervision administrations, as well as relevant central enterprises: On April 16, 2014, around 10:00 a.m., a serious dust explosion occurred at Rugao Shuangma Chemical Co., Ltd., located in Dongchen Town, Rugao City, Nantong City, Jiangsu Province. The incident took place during normal production operations in the stearic acid granulation tower. Maintenance workers were installing a gas oscillator outside the warehouse and welding a support frame for an atomized water pipe when an explosion of stearic acid dust occurred. The accident resulted in 8 fatalities and 9 injuries. Shuangma Company was established in June 1997 and currently employs 246 people. It mainly produces and sells stearic acid and glycerin, with an annual capacity of 100,000 tons of stearic acid and 10,000 tons of glycerin. The company uses imported palm oil as raw material and processes it through catalytic hydrogenation, high-pressure hydrolysis, distillation, refining, and granulation. The explosion occurred in a multi-story steel structure workshop housing four granulation towers, each with a diameter of 4.5–7.0 meters and a height of 27 meters, installed on a 5-meter steel platform. Stearic acid is sprayed from the top and cooled by counterflow with gas from the bottom to form particles. According to preliminary investigations, the direct cause of the accident was that the company failed to stop the material emptying process before installing gas oscillators and performing welding work on the atomized water pipe support frame, leading to the accumulation of stearic acid dust and subsequent explosion. The explosion caused structural failure and collapse of the granulation tower. Further investigation is ongoing to determine the full cause. Following the incident, senior officials emphasized the importance of rescuing the injured and strengthening safety measures in high-risk industries. The State Administration of Safety Supervision has instructed relevant authorities to investigate the cause promptly, report findings transparently, and prevent similar incidents. In response, the following measures are proposed: **First, strengthen the main responsibility of companies for safe production and improve dust site management.** Enterprises must fully recognize the risks of dust explosions, implement safety regulations, conduct risk assessments, ensure regular inspections, and maintain proper ventilation and dust removal systems. Employees should be trained in safety procedures and emergency response to prevent accidents. **Second, enhance the management of hot work and other direct operations.** Regions and companies should strictly follow safety regulations, conduct risk analyses before any hot work, clean equipment thoroughly, and ensure proper controls are in place to prevent dust explosions. All operations must be approved and monitored to avoid accidents. **Third, continue to identify and address hidden dangers.** Companies should systematically assess dust-related risks, test combustible materials, and develop targeted safety measures. Hazardous chemical companies must follow guidelines for hazard identification and management to ensure continuous improvement in safety practices. **Fourth, learn from this tragedy and reinforce safety oversight.** Authorities should intensify training, increase inspections, and ensure strict enforcement of safety laws. They should also promote standardization in chemical processes and hold responsible parties accountable for violations. This notice is issued to ensure all related companies are aware of the incident and take necessary actions to prevent future accidents. **Please distribute this notice to all relevant companies.** **National Security Administration** **April 23, 2014** *This article is reprinted from online media.
